1.先問history(最重要的是Onset的時間，3小時內要打t-PA，6小時內(我們醫院4小時內是極限)要做thrombectomy)，original status要搞清楚(搞清楚是新的中風還是以前就曾經中風過)
2.PE and then NE(Consciousness, cranial nerve, muscle power, coordination, sensory, gait)
1b:
- 失語症和木僵的病人無法理解問題時計2分
- 昏迷的病人（1a＝3）計2分
- 其他非失語症問題如因插管、口 腔氣管外傷、嚴重構音困難、或語言問題（外語、方言等）而無法回答計1分
1c:
- 執行命令，1.閉、開眼， 2.握、鬆拳（好邊的手）
- 若病人嘗試做但因無力而做得 不完全，仍給分
- 如果病人沒反應，就做給他學
- 只評估病人第一次的嘗試
- 若受測者因外傷、截肢或其它身體的問題無法執行上述命令 時，可以用其它簡單命令取代。
- 昏迷的病人（1a＝3）計2分
2:
- 只測水平方向的眼球運動，若受測者眼球無法自主運動時， 可轉動受測者頭部來引發眼球運動(oculocephalic reflex)
- 若病人兩眼持續看向任一側 (conjugate deviation)，但 仍有自主或反射性眼睛運動， 計1分
- 若病人有單眼運動神經（CN III,IV,VI）病變，計1分
5-6
0 = 正常:持續10秒不墜 1 = 輕癱:10秒內手臂下滑， 但未完全落下 2 = 能抬起，但無法抬至 90(或45)度 3 = 僅能水平移動，無法抬起 4 = 完全癱瘓
7-8
0 = 正常:持續5秒不墜 1. = 輕癱：5秒內腿下滑， 但未完全落下 2. = 僅能抬起對抗重力，無 法抬至30度 3. = 僅能水平移動，無法抬 起 4 = 完全癱瘓

- 請病人讀或複誦。不要告訴病人測試的目的
- 如果病人有嚴重的失語症，依其自發的言語清晰度加以評分

- 從前面的檢查可以得知是否 有忽略
- 若病人有失語症，但表現出可以注意兩側，計正常
- 若病人出現視覺空間感忽略 或無病識感，計不正常
Plasmin is the enzyme that degrades fibrin, the protein which is the main constituent of blood clots
rt-PA activates the release of plasmin as plasminogen
1.只有blood sugar 可能會影響打或不打rt-PA
2.EKG, Troponin I, Chest x ray: 不要因為這些檢查而delay打t-PA的時間
3.等最久的通常是抽血，做brain CT 之前就先把寫抽掉送出去
Dense MCA sign
1.This is a result of thrombus or embolus in the MCA.
Insular Ribbon sign
This refers to hypodensity and swelling of the insular cortex.
It is a very indicative and subtle early CT-sign of infarction in the territory of the middle cerebral artery.
This region is very sensitive to ischemia because it is the furthest removed from collateral flow.
It has to be differentiated from herpes encephalitis.
